About ACC
If you have an accident or get injured, ACC helps pay for your Physiotherapy treatment.
It doesn't matter if your injury happens at work or away from work, at home or when you're playing sport. You can still get help from ACC.
An accident is defined as a sudden event, so you must be able to name the time and place of injury.
What to do to make a claim:
Step 1:
Visit your Physiotherapist
Step 2:
Fill out the ACC45 claim form (available at your Physiotherapists surgery) with your Physiotherapist
Step 3:
Your Physiotherapist sends the form to ACC for processing
Step 4:
ACC will contact you about your claim to let you know if it has been accepted or declined
What do you have to pay?
As ACC only pays a portion of the full cost of your physiotherapy treatment, a co-payment as outlined above needs to be paid at the time of each treatment. Please note that a non-attendance charge equivalent to our full private fee will be made where an appointment is missed without any prior notice.
